The processing fee is $25.00 per applicant, and applies only
to new passport books and cards for adults and minors. If
applicant is applying for both the book & card simultaneously,
there will be only one $25 processing fee.
The cost of pictures taken in the County Clerk's Office is
$15.00 per applicant, PER EACH BOOK OR CARD.
County Clerk fees are payable by cash or check made payable
to ‘Rockland County Clerk'.
*PLEASE NOTE: THERE ARE NO PROCESSING FEES FOR PASSPORT
BOOK RENEWALS OR FOR ADULT PASSPORT CARD APPLICANTS ALREADY
HOLDING A VALID PASSPORT BOOK.
What must I provide with my DS-11 application?
a. Official birth certificate with a raised, embossed or
multicolored seal, or previous passport, or original Naturalization
or Citizenship Certificate. (Minors under 16 must present
a birth certificate listing the names of both parents and
their old passport.)
b. Official picture identification (ie: Driver's License).
c. Two passport pictures.
d. The applicant's social security number.
e. A check or money order as described in question 3 below.
If the applicant is 18 or older but does not possess a Driver's
License, an identifying witness must appear with the applicant.
The identifying witness must have a valid Driver's License
or Passport, and the applicant must have an item from 4(a)
above and four to five (4-5) other I.D.'s ie: School I.D.,
medical or Social Security Card.
IF THE APPLICANT IS UNDER THE AGE OF 16, BOTH PARENTS MUST
PRESENT VALID ID AND APPEAR WITH APPLICANT, PRESENT AND
BE LISTED ON THE CHILD’S BIRTH CERTIFICATE, PRESENT
THE CHILD’S OLD PASSPORT, AND SIGN AND TAKE THE OATH
BEFORE ACCEPTANCE AGENT. IF ONE OF THE PARENTS IS UNABLE
TO APPEAR, THE APPEARING PARENT MUST PRESENT: EVIDENCE THAT
HE/SHE HAS SOLE LEGAL CUSTODY OF CHILD (CERTIFIED COURT
ORDER OR DEATH CERTIFICATE), OR, WRITTEN NOTARIZED CONSENT
OF OTHER PARENT FOR ISSUANCE OF PASSPORT, OR, WRITTEN NOTARIZED
STATEMENT EXPLAINING WHY NON-APPLYING PARENT'S CONSENT CANNOT
BE OBTAINED.
IF APPLICANT IS 16 - 17 YEARS OF AGE, ONE PARENT MUST APPEAR
WITH THE APPLICANT. ALL APPLICANTS REGARDLESS OF AGE, MUST
APPEAR.
When will I receive my passport?
Your passport will be processed at one of the nationwide
passport centers and forwarded to you by one of these centers
within approximately 6 - 8 weeks. Processing times vary
according to the time of the year.
What if I need it sooner than 6 - 8 weeks?
If you will be traveling in 3 - 6 weeks, we suggest that
you consider sending your passport via U.S. Express Mail
and submitting an additional fee of $16.25 for Express Mail
return and/or utilizing the Expedite Service ($60.00 extra
charge per applicant). If you are traveling in less than
21 days, we suggest you use both the Expedite Service and
overnight mail both ways. You will receive your passport
in approximately 2 weeks.
However, if you go directly to the Norwalk or NYC Passport
Offices, you may get your passport on the day you apply.

Where can I get my pictures taken?
Pictures can be taken in the Rockland County Clerk's Office,
or at locations offering "official passport pictures".
Passport pictures must comply with strict U.S. Passport
Services requirements.
Will documents submitted with my application (ie:
old passport or birth certificate) be returned to me?
With the exception of affidavits done specifically for application
purposes, these documents will be returned with your new
passport.
How can I amend or add more pages to my current
passport?
To amend your passport (THAT IS LESS THAN 1 YEAR OLD), fill
out the DS-5504(Passport Reapplication Form) If passport
is over 1 year old, applicant must submit DS-82 and pay
normal fees. Customers needing to add extra VISA pages to
their passport must submit form DS-4085 (Application for
Additional Visa Pages) This service is free of charge.
How do I get my birth certificate?
Birth certificates can be obtained at the Village Clerk
in the location where the birth took place. For births in
New York State, but outside of New York City, they can also
be obtained by calling (518) 474-3077. For births in NYC
call (212) 788-4520.
